Q : Do you have a failed transaction complaint on your UBA Card?

- Click here to download the attached form to report the dispense (ATM) error or refund for any charge on your UBA Card for which you did not get value (Charge back).
- Complete the form and drop at our nearest UBA branch to you OR
- Send a mail to cic@ubagroup.com and including the following details in the mail:
- Account Number
- Account Name
- Card Number (first 6 digits and last 6 digits)
- Mobile Phone Number
- Transaction Amount
- Date and Time of Transaction
- Charge back/dispense error reason given
